Terms and Conditions: By completing this form, I certify that I am 21 years of age and agree to the terms laid out. I authorize Eleven Eleven to charge my credit card for the wines, tax, and shipping charges associated with my Club 11:11 shipments and/or other wine orders I may place. Members are responsible for all taxes and shipping fees, including shipments that are refused, returned or shipped to an old address. Should your club shipment be returned for any reason, the fee to re-ship would be the full rate charged by the common carrier to Eleven Eleven. I understand that I have committed to receiving a minimum of two shipments and that membership will continue until I notify Eleven Eleven to cancel. Cancellations must be received via email or phone prior to the billing cycle for the next future shipment. A signature of a person 21 years or older is required at the time of delivery.
