Terms and Conditions: By signing this form, I certify that I am 21 years of age and agree to the terms laid out. I authorize Eleven Eleven to charge my credit card for the wines, tax, and shipping charges associated with my Club 11.11 shipments and/or other wine orders I may place. Members are responsible for all applicable taxes and shipping fees, including shipments that are refused, returned, or shipped to an old address. Should your club be returned for any reason, the fee to re-ship would be the full rate charged by the common carrier to Eleven Eleven. I understand that I have committed to receive a minimum of two shipments and that membership will continue until I notify Eleven Eleven to cancel. Cancellations must be received via email or phone prior to the billing cycle for the next shipment. A signature of a person 21 years of age or older is required at the time of delivery.
